Once you are booked in, you will receive an email confirmation of your appointment. If this is your first time at our clinic, please fill out the intake forms by following the link in your email, or download the forms.
If you are not able to print the forms, please arrive 15-20mins early to complete them before your visit.
Please bring along the following to your initial appointment:
• Any copy of lab work or imaging you might have from the past year
• A list of medications or supplements you are currently taking

Set up a meet and greet with your desired healthcare practitioner to find out if they’re right for you. If you don’t know who to see, one of our team members will suggest the best match based on personality and health concerns.
